Tibe2 rase nak makan sushi. Hmm..sedapnye..

Tinggal satu minggu lagi kat MGH.
Tinggal enam episod je sebelum YAB abis.

Ni yang rase nk buat skendel musim sejuk lg ni. Hahaha~

Tapi kalo ikutkan since second year, mmg every winter mesti ade skendel. Takbleh blah tul. Membuatkn aku rase dreadful gle stiap kali winter datang. Haishhhhh! Ble takde skendel br ni, mula la nk igt skendel2 lame. Ape2pn aku harap mereka(i.e. mangsa2 skendel saye/saye mangsa skendel mereka??) berbahagia selalu buat jahat dibalas jahat, ingat yee..

Aku mmg selalu doakn yg terbaik utk sume orang, tak kira la yg jahat atau baik(uwek uwek). Wohoho..tibe2 terigt ayat diri sendiri kepada Encik Baju Biru (skendel lame yang hampir berputik smule), "Bless my kind heart..I know I am such a good person". Kalo kitorg duduk kat dunia anime musti dier dah tergolek jatuh dari tempat duduk dier dan gelak guling2 ble dengar ayat tu. Haha... Tapi kat alam dan manusia ni, dier cume gelak dengan sopan santun je la.. Hei Encik Baju Biru, saye mmg baik ape! At least saye takla mengaku saye suke awak pastu peluk awak sampai awak rase serba salah. Ok la tuh sebab saye cume imagine saye buat camtu jer. Waaahh meliarnye imaginasi..

Mari Berbuka

Berjaya membuat sate goreng itu dengan jayanye. Sedap. Bleh buat lagi. Hoho. Sebab takde timun, maka improvise dengan salad ngan cherry tomato. Sedap juge.

Sabtu ni plan nk ke Connemara pulak. Si Katha sudah pn menyewa kereta. Tolongla jangan lupe bateri lagi!

Semalam jumpe sorang makcik ni...current diagnosis Bulbar Palsy. From someone who's perfectly articulate, she becomes dysphasic. If the cause of her BP is Motor Neuron Disease (MND), then maybe she has only 2-3 years to live.

We just diagnose another person with MND last week and since his disease is more progressive the SPR predicts he has less than 18 months to live.

Seen too many patients with MS too...

Not forgetting those with peripheral neuropathy.

And yeah...the rare limbic encephalopathy +/- temporal lobe epilepsy.

Oh..stiff person syndromes was awesome!

Then we met this uncle who has leg cramps like every 2 minutes! (when I failed to finish the neuro exam, the neuro SPR was like," was it even a true spasm?". Then he looked at me with this expression on his face ---"you're just lazy, haa haa". Whatever. Nasib baik (could I say this to that poor uncle??) the spasms also occurred when the SPR and I went back to see the patient together. The SPR was like, "ohhh this is so wonderful, I don't know whats wrong but this is unbelievable! He really got cramps!". Yes Doctor, I wasn't lying or being lazy when I said he got leg cramps!!!

...and finally one pakcik that receives full TLC (tender loving care) since he's unresponsive - Dx of paraneoplastic syndromes. =(

Ingat buat neuro jumpe stroke patients je. Man..I was so WRONG!

Last Sunday I bought a new wrist watch because I had accidentally(??) left the old one in a mosque in Germany. Poor thing, I wonder who has it now. Hopefully someone is taking good care of it. Anyway, thanks Idah for accompanying me across the town, helped me to choose one out of hundreds watches which is painful enough and finally for swiping yer CC without any second thoughts! One thing I realized is that I am very fond of men's watches. Seriously, theirs are waaayyy better designed than women's. Cool. Macho. Do I like being macho? Yes, most of the time I do =) Anyway, mine look more or less like this one. Sleek, smart and sangat macho kan? Plus I can change the strap to either black or brown. Convenient! Weeeee~

Najmi and I were going through psych problem based learning (PBL) assignment just now. The patient was presented with few symptoms that is suggestive of schizophrenia. While reading about other positive schizo symptoms mentioned in the book, we've had encountered the word 'gustatory' hallucination and wasn't sure what does gustatory mean. Using one site called 'URBAN dictionary' ( ni Najmi la pakai ni) we got this definition:

so you can almost guess what were our reactions next.

We've had the first video session today. It's distressing enough to watch a playback of yourself conducting a session of history taking - and it's more disturbing when there are 13 other people watching it together with you! Ughhhh...eventhough the Doc says we all did an excellent history taking I can't help but to be more abusively critical towards myself considering the fact that I know I could've done better than what I already did. Hmmm...I sometimes hate the fact that I would be overly obsessed with perfection.

Crazy Beautiful

Took my first psychiatric history today. Thank god Navid was with me or else there would be A LOT of awkward silence between me and the patient. She suffers from depression. She told us that most of the times she felt worthless and even at one point tried to kill herself by overdosing on her meds. Oh my, she is such a beautiful girl. On a few occasion, I wasn't even listening to her talking. All I could think of is how beautiful she looks.

Anyway, the moral of the story is there's nothing perfect in this world. You could look pretty from the outside but pretty rotten on the inside. Jangan menilai buku daripada kulitnya ya kawan-kawan!

But this lady is so nice that Navid and I both think she doesn't deserve this illness(ohh seperti menolak takdir pula) =(
Poor lady, hope she'd recover soon.

Oh ya, dua sebab untuk saya menjadi sangat elated today:
1) Saya akan terbang jauh esok, selamat tinggal Galway! =)
2) Hanya beberapa orang yang tahu sebab kedua, tapi jangan cuba korek rahsia ini dari mereka. Awas!

Hehe..happy weekend everyone, mine starts tomorrow and it ends on Monday.

Too Much Time to Be Wasted (not)

Okay, I've got to be honest. I am actually missing those pregnant bellies! Seriously, how will I cope with the mental wards, surrounded with depressed people and panic buttons(just in case). Can I please push MY panic button so that everyone will come and help? Teehee.. Send me over to those midwives, I don't mind pulling some hairs to get 100 more deliveries.

Intan said I can ask about our status indirectly. I am getting anxious as the text are getting more frequent. What does that mean? But to ask that person directly/indirectly...alamak maluuuuuuuuunye ("ohhhh..i feel stupid talking in bahasa!" *quote* Bwahahaha!!) Should I just keep this anxiety in my poor tummy?

This part of the world is getting colder and colder each day. I think I am going down with flu. Sore throat, aching limbs, high temp? Nooooooo..I want to go to Germany this Thursday - and I shall even if I had to wrapped myself in this thick duvet and board in that flight! Boohoooo =(

Isyqi, I am coming no matter what! And Azraai, you will get those flipflops as promised! (very determined)

Ohh..mahukah sesiapa pergi ke town bersama saya esok? *beating eyelashes*

I'll be having my OSCE exam on Friday, 935am. Wish me luck, I need loads of 'em.

Genital warts are NOT something you want to get-ever. Seriously. GROSS. Okay, this is random enough, but when you've seen one, you'll know.

Ohh, the cardio exam with Dr. R was fUntastic. The patient was nice too! Hehehe...and Dr. Sarah is an absolute legend; although she was late for the cardio- exam-bedside -teaching-thingy, she called Amira's phone to apologize! Some people just don't care. She even gave us quick tutorial about catheters, stoma bags and NG tube though she was busy - awhhh.. Infectious disease team are just wonderful!

So What?

Oh, I am Still Not over Him - YET. But I am sure I will, someday. =)

Anyway, still struggling with everything in school.

Currently I am so angry with myself for being super LAZY. I wouldn't even walk to the bathroom though my bladder is super FULL.

Hopeless case. I need an energy booster of some sort.

I can feel no emotion towards the upcoming OCSE exam. Owh, but today I learnt about vein guttering and its relations with incompatible arteries. Yeay! Thanks Idah for pushing my lazy brain to think!

Had to answer Sarah's (the intern) bleeps (twice!) and I like it! Honestly, I am SO a professional - someone could hire me as secretary. Teehee..

Emotional.

I was in plastic Outpatient today and spent most of my morning admiring Dr. John- I can pretty much say at this point that I am kind of attracted to him. He is so COOL and RELAX - which is totally opposite of my "ALL OVER THE PLACE" nature- people do say OPPOSITE ATTRACTS. Point taken. He jokes around when I least expect it; hehehe..totally funny. (I LIKE!)

Then I decided to follow Mr McCann for the rest of the afternoon alongside five other students plus an intern (!!) in and out of four different rooms in the Outpatient department. Mr McCann loves having students trailing behind him as he attend to each patients; plus he really is a GREAT teacher. He teaches a lot-and asked A LOT of questions too! Heh.

During one of the Q&A sessions, my brain was forced to come out with some answers and he wouldn't take "I can't remember" as a reply. My face was blushing hot as 10 pairs of eyes (including the patients') was looking straight at me, waiting for me to explain about "TRIGGER FINGER". Glad I'd got over it, still fully intact...though my ego was a bit bruised -but nothing major that I can't bounce back from! (at the end of the day 'Seborrhoeic keratosis' gave me a reason to smile!)

At one point, Mr McCann was attending to one little boy; about 6-7 years of age. He had his left ear bandaged and as well as his upper lip and nose. As Mr McCann took off the bandages, the deformities made clear. He had his cleft palate repaired and possibly an insertion of a small plastic ventilation tube in the eardrum to allow proper drainage and air circulation as cleft can interfere with the function of the middle ear. When the bandages were off, the boy asked if he can see himself in front of the mirror. Mr McCann said, "Go ahead"- and I could never ever forget the expressions on his face as he sees the mirror for the first time. He looked so RELIEF- constantly moving his head up, down and to the right side while checking his well intact left ear, nose and lips ; he looked SURPRISE too, but most importantly was he looked so HAPPY! He joyously said "THANK YOU!!" to Mr McCann. Awhhhh..indescribable feelings of warmth suddenly rushing within and I feel like crying.. at that moment I am so inspired to become a plastic surgeon. Hehe..

Met A LOT of really GOOD LOOKING patients again this week. OMG, seems like plastics outpatient clinics are always full of them. Mind you, they are not in for any plastic surgery, face lifts or anything like that! One had splinter on the right arm of which need the bandages changed , one had keloid scar on the chest who wants it removed, one had some kind of accident involving his right index, middle and ring fingers which need the be x-rayed and reviewed. There you go, full report of my outpatient activity for today. :)

So Much, I Learnt

I'd learnt so much today; the doctors are so well trained and they had been setting up really good examples that are so useful for our future practice; for example:
1) NEVER ask for patient's consent
2) NEVER listen to the patients, besides they knew NOTHING right?
3) HELP SPREADING MRSA by touching everyone and everything and DON'T BOTHER to use the available alcohol gel to disinfect your hands
4) FORCE patients to comply on physical examination even when they said they're in excruciating pain
5) POKE, PRODE and HUMILIATE patients like they're dummies for learning purposes or just useless broken things that were sent to be fixed by remarkable doctor's skills, thus satisfy our ego
6) JUST LEAVE the patient in pain and head on to the next one!


Now, where did I put my Tally and O' Connor book...?

I went shopping alone in Dunnes early this evening. Found myself struggling to resist the temptation of buying those half prized chocolates which are neatly arranged-color, size and prize coded..hehe...seems to me like they are. A few minutes later, I found myself running away from the chocolate isles, empty handed. Obviously I manage to get a grip of reality, thank God-I am gaining up on some few kilos-which I NEED to shed quickly.

On the way to and from Dunnes, I had to face another temptation. One single house that I apt to give a quick peek each time I walk pass by. Proudly saying I wasn't looking at it at all. It's so unusual when something as simple as this gave me so much satisfaction.

Plastics Outpatients today with Mr. McCann - I met three guys suffer from partial, and deep burns. Two of them were the victims of plane crash in Phuket few months back (remember it was in the news??). I've read about the accidents a few times before, but I never thought that I would meet the survivors in person. One of them is SO GOOD LOOKING!- but doctors should have nothing to do with patients. Full stop.

Looking at too much "Dupuytren's contracture" - I can't help but to remember the first time I've ever heard of such a thing, and think..I was so young and naive, and such a fool! =)

B.L.A.N.K.

I am so tired as the weekend approaches that I've started to feel annoyed with everything and everyone. Even most of the patients are cranky because all the 4th and final meds are constantly swarming around the hospital. For example, our infective endocarditis patient today. He was so annoyed to see us coming that I could see "MEANness" written all over his face...yeah, like really all over. He wouldn't even force a smile. But the good thing was, as I laughed at all his MEAN and INAPPROPRIATE jokes he finally gave up and co-operate...and finally I could listen clearly to his pan-systolic MURMURed heart! Yeay!

Dr Niamh!

She's the A&E consultant now! I can't believe it and she still remembers us!! Back in A&E for a while today..met Dr Matthew~ Well, the fact that I can never function well in front of him- well, at least I manage to get the blood in the tube this time....Grrrrr...I am always appear inadequate whenever he is looking. Thank god my heart wasn't thumping or else I'll be poking the artery instead. Hah...he is as cute as ever, with that devilish smile...hmmm.

p/s: he said I could come back anytime to practice my "blood-skill".....aaawwwhhh...he'll be my fav doc 4ever!

A Reason To Be Happy.

For two nights in a row, I've been feeling a little bit upset because of my inability to put an I.V. catheter into patient's vein. First time I tried was on Wednesday night and I missed the vein by a few micrometers and punctured the patients' muscle instead. No blood came out, of course. In the end, Dr. Matt had to put it in himself. Thank God, the patient wasn't upset or anything, but I can see that Dr. Matt was a bit disappointed. Last night I tried again, but that patient is too old and usually old person has 'weak veins' and are easily ruptured (Dr. Sarah taught me this). I got the vein all right, but because I was trembling vigorously, I managed to rupture it too. When the lab result came out, the reading was totally screwed because the blood sample was hemolyzed. I was so upset with my inadequacies that I wasn't able to face Dr. Matt- at all. So, like usual he had to cover up my mistakes (again) and drew out the blood himself. *tears* I am sorry Dr. Matt!!!!

This morning, I was hanging around Dr. Sarah as usual. We met this patient with a probable appendicitis and Dr. Sarah look at me in the eyes and ask, " Do you want to do her blood?". I was reluctant to say yes because I've started to think that I will never do it right. So, my brain instantly screaming, "No Farah, do not agree to this offer. You will never do it right. You're just going to hurt another patient today. Do you need to be disappointed for the third time?". Guess what, I said "Yes" to Dr. Sarah's offer- instantly. It's like my mouth just ignoring the "NO, THANK YOU" signal sent from my brain. I almost kick myself for agreeing.

Actually, I did it right this time. Alhamdulillah. Blood was oozing smoothly out from the I.V. catheter and into the syringes and I just couldn't stop smiling. Dr. Sarah praised me a few times for my success. And nothing, that anyone could say to me today that can undo that happiness~~

Ohhhh Dr. C!!

The ward round this morning was all about attitude and knowing who's the boss. Watching Dr. F nonchalantly giving orders to his colleagues; I was thinking,"Wow, this man is not a person you want to mess up with". According to a few final meds yesterday, he is very good and really knows his stuff. These 'very good' and 'really knows his stuff' part came with a package unfortunately, it's called 'attitude'. Hello Mr. 'Don't Bleep Me Twice'! Ohh well, he is kind of tough sometimes, but I really like the way he gave Amira and I our own 'private' tutorial this afternoon. The final meds are really going to kill us this time. Yesterday, we were after the same doctor, i.e. Dr. F to give us urology tutorial. Dr. F brushed them off, because he wanted to give us the tutorial - he said that he will give them a tutorial the next day i.e. today. And apparently, today he brushed them off again to next week's tutorial and claimed that he's super busy. But when Amira and I went to see him after that, he immediately agreed to spare us some time to listen to our history taking. Hmmm...I wonder if.... :) Hopefully those final meds will eventually get their tutorial as promised.

Anyway, we were in the theater today with Dr. C! He is SO CUTE! I am so in love with his puppy eyes. I still can remember that look on his face when he was being scolded by that 'granny' nurse for leaving his white coat by the window. She obviously been nasty to everyone. Hello Madam 'Not More Than Three Students In The Theater Next Time'! OMG that was annoying! Okay, our fault for not sorting things out among us. But I think she shouldn't be so nasty with Dr. C. One time during the cystoscopy, she was saying something and Dr. C replied. To my horror she said, " I wasn't talking with you, I was talking with the nurse" plus she was making faces with the doctor! OMG! Whatever!!

Okay, overall I'd say I've enjoyed these two days so far. Lots of drama, lots of action in you know what department. Hahaha. Remember folks, take care of your bladder and kidneys and hopefully you don't have to see us. Or shall I say; take care of your bladder and kidneys so that we don't have to see yours-know-what. :p

p/s: I randomly wrote poems like the one in the previous entries, nobody has to feel attach to it in any way, okay!
